Replacement Cards

The fee to replace a current or expired certification card is $20.00.  This replacement fee applies to all certification cards requested 30 days or more after the certification course completion date.

 

NOTE: Replacement cards are available only if you attended a certification class in Region 20. If your certification class did not take place in Region 20 please contact the region which provided your certification class.

NOTE: IF YOUR CERTIFICATION IS MORE THAN ONE (1) YEAR EXPIRED, YOU WILL NOT BE ELIGIBLE FOR A RECERTIFICATION CLASS. YOU WILL NEED TO RETAKE THE INITIAL CERTIFICATION CLASS.
